An established and well‑respected aged care organisation on the Central Coast is seeking an experienced Nurse Practitioner (Aged Care) to join their clinical leadership team. This is a stable, resident‑focused role offering autonomy, meaningful impact, and genuine work–life balance.


About the Role:
You will work across co‑located residential aged care homes, providing advanced clinical care, supporting early intervention, and strengthening clinical outcomes. This role is ideal for an NP who enjoys a blend of hands‑on clinical practice, decision‑making autonomy, and collaborative work with GPs, families and internal nursing teams.

Key Responsibilities
  • Conduct advanced assessments, initiate diagnostic pathways, and develop clinical management plans for residents with complex or chronic needs.
  • Prescribe and review medications, lead clinical interventions, and coordinate with GPs and allied health for integrated care.
  • Provide clinical leadership, mentoring, and support to the nursing team, helping uplift capability and clinical standards.
  • Collaborate closely with families and residents to ensure care aligns with goals, preferences and comfort.
  • Support palliative and end‑of‑life pathways, ensuring high-quality, compassionate care.
  • Contribute to quality improvement initiatives and clinical governance activities across the sites.
What We’re Looking For
  • Current AHPRA Endorsed Nurse Practitioner.
  • Experience in residential aged care, gerontology, or complex chronic care.
  • Strong diagnostic reasoning, assessment, and prescribing capability.
  • Excellent communication skills with the ability to build strong relationships with residents, families, and clinical partners.
