You’ll work in a modern clinic co-located with Residential Aged Care Facility, offering the perfect blend of general practice and continuity of care for aged care residents. From day one, you’ll have a full patient load and the chance to make a real impact in the lives of our community members.
- Position: Nurse Practitioner
- Role: Full time
- Setting: Clinic- Primary Care/ Aged Care
- Independently diagnose acute, episodic, and chronic illnesses, providing timely and effective management plans tailored to each patient’s needs
- Request and interpret a variety of diagnostic investigations, such as pathology (blood tests) and radiology (X-rays, imaging), to inform clinical decision-making and guide treatment
- Prescribe, review, and administer medications according to approved formularies, ensuring safe and evidence-based pharmacological interventions
- Refer patients to appropriate health professionals, including GPs, specialists, and allied health providers, and coordinate care to ensure seamless transitions and continuity of care
- Facilitate and participate in advance care planning discussions with patients, families, and the healthcare team to ensure patient wishes are respected and documented
- Adhere to relevant legislation, clinical guidelines, and professional standards, and engage in ongoing professional development and reflective practice to maintain and advance clinical expertise.
Key Requirements:
- Current AHPRA registration with Nurse Practitioner Endorsement
- At least 1 year of experience as a Nurse Practitioner (Primary Care/Community Care/Aged/CDM/ General)
- Strong clinical assessment and decision-making skills
- Comfortable with electronic medical records and clinical software
- A collaborative, team-oriented approach
