Sharp & Carter is partnering with a community focused clinic located in the beautiful Western Suburbs of Melbourne—approximately 70 km from the CBD, 35 km from Ballarat, 50 km from Werribee, and 44 km from Gisborne. This unique position combines general practice with continuity of care for aged care residents, as our clinic is co-located with a Residential Aged Care Facility.

Key Responsibilities:
  • Independently diagnose acute, episodic, and chronic illnesses, providing timely and effective management plans tailored to each patient’s needs
  • Request and interpret a variety of diagnostic investigations, such as pathology (blood tests) and radiology (X-rays, imaging), to inform clinical decision-making and guide treatment
  • Prescribe, review, and administer medications according to approved formularies, ensuring safe and evidence-based pharmacological interventions
  • Refer patients to appropriate health professionals, including GPs, specialists, and allied health providers, and coordinate care to ensure seamless transitions and continuity of care
  • Facilitate and participate in advance care planning discussions with patients, families, and the healthcare team to ensure patient wishes are respected and documented
  • Adhere to relevant legislation, clinical guidelines, and professional standards, and engage in ongoing professional development and reflective practice to maintain and advance clinical expertise.

Key Requirements:

  • Current AHPRA registration with Nurse Practitioner Endorsement
  • At least 1 year of experience as a Nurse Practitioner (Primary Care/Community Care/Aged/CDM/ General)
  • Strong clinical assessment and decision-making skills
  • Comfortable with electronic medical records and clinical software
  • A collaborative, team-oriented approach
