About the Role
Due to ongoing growth, we are seeking an experienced and motivated Occupational Therapist to join a dynamic community aged care services team in the Western Suburbs of Melbourne.
In this community-based role, you will work closely with older adults and their families to support independence, safety, and positive rehabilitative outcomes at home. Working as part of a multidisciplinary team, you will:
- Assess functional ability and identify opportunities for intervention
- Collaborate with consumers and Care Advisors to set meaningful goals
- Deliver practical interventions, including home modifications and assistive technology, to help clients live well and maintain independence
Key Requirements
- Current registration as an Occupational Therapist with AHPRA
- NDIS Worker Screening Check, current First Aid & CPR certificates
- Experience in occupational therapy assessments and interventions with individuals and families
- Ability to provide interventions to maximise independence and quality of life
- Strong communication, organisational, planning, and time management skills
- Proven ability to build effective working relationships internally and externally
- Strong IT skills, database experience, and a current driver’s licence
- Member of Occupational Therapy Australia
- Knowledge of Home Care Packages and aged care community services
- Experience with brokerage models of service and Consumer Directed Care
- Familiarity with Carelink Plus
