The Position
You will partner with clients to ensure the services they need and want are provided via Case Management assessment. Key responsibilities;
- Visit and interview clients
- Assess client circumstances
- Determine eligibility for care services
- Develop care plans to support client
- Case management experience
- Proven skills and experience in the above duties
- Qualification in Community Services or Aged Care
- Be resilient, a problem solver & relationship builder
- Current driver’s license
- Self-motivated, positive attitude and caring
